Community Health Worker

Position Summary Community Health Workers (CHWs) assess and address clients' social needs, developing plans to connect them with resources to mitigate Social Drivers of Health (SDOH) challenges. CHWs engage directly with Older Adults at partner organization sites and, when necessary, in clients' homes, providing support and resources to improve SDOH-related adversity. Working under indirect supervision, CHWs coordinate with medical providers, primary care teams, and social services to deliver short-term care coordination and educational support, improving clients' health and well-being. CHWs work in both clinical and community settings, and, when applicable, may offer peer support if they have personal experience in mental health or substance abuse recovery. This is a term position (renewable up to four years) training position in collaboration with the Senior Employment Program (SEP) of the New Mexico’s Aging & Long-Term Services Department, which supports the “Health Equity and Social Connectedness for Older Adults” programs. As part of the program, the CHWs are hired and staffed by partner organizations who receive funding support (including the cost of employing the CHW) as part of the program. It is half-time, twenty (20) hours per week. Training to become a Certified Community Health Worker will be provided as part of the program, with certification costs covered by the employing organization.

Conditions of Employment
  • Possession of a valid New Mexico driver's license is a requirement for this job.
  • Must pass a pre-employment criminal background check.
  • Fingerprinting, and subsequent clearance, may be required for this position depending on assignment.
  • May be required to obtain a Community Support Worker (CSW) certification within six months of hire.
  • Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.
  • Successful candidate must submit to post-offer, pre-employment physical examination and medical history check.
Minimum Qualifications

High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Preferred Qualifications

 

  • Strong interpersonal and communication skills with the ability to work effectively with a wide range of people in a diverse community.
  • Knowledge of community agencies and resources.
  • A comprehensive understanding of Social Determinates of Health.
  • Working knowledge of social service programs and health-related services.
  • Ability to plan, implement, and evaluate individual client care plans.
  • Knowledge of transportation and other barriers to care that may be encountered by clients.
  • Ability to communicate medical information over the telephone to healthcare professionals and care coordinators.
  • Skill using personal computers and related software applications, including email.
  • Skill in organization of resources and establishing priorities.
  • Creative and analytical thinking.
Additional Requirements According to the contract requirements, candidates selected for the position must be at least 55 years of age or older to effectively address statewide Social Determinants of Health (SDOH) for older adults.
